At the 2nd Scar Treatment Technology Advancement Conference, Dr. Wang Liang from Bakaang Medical Wuhan Branch gave a special presentation on "Techniques and Management of In-Situ Autografting for Keloid Tissue." He shared Bakaang's clinical experience in this technique, covering aspects such as surgical methods, fixation techniques, and postoperative management.
What is in-situ autografting for keloid tissue?
In simple terms, it involves retransplanting the excised keloid tissue back to its original site after processing.
The advantages of this technique include:
Use of autologous tissue, eliminating the risk of immune rejection;
No need to harvest skin from other normal areas, using keloid tissue instead, turning waste into a valuable resource;
The keloid tissue retains some fibroblasts and growth factors, which promote local repair and vascular regeneration after reimplantation.
The surgery involves three steps, and Dr. Wang Liang introduced the basic operational procedures:
Excision of the keloid: Complete removal of the lesion.
Preparation of the graft: Trimming the excised keloid tissue to the appropriate thickness and size.
Fixation: Using conventional pressure bandaging, packing, or negative pressure fixation to secure the graft to the wound surface.
After the graft takes (usually 15-20 days for wound healing), patients need to undergo superficial radiotherapy, and some may require additional treatments such as fractional laser, pressure therapy, or medication.
Dr. Wang Liang emphasized that the treatment of keloids cannot rely solely on surgery. Postoperative management and long-term follow-up are crucial for reducing the risk of recurrence. Bakaang has accumulated years of experience in follow-up, using regular imaging comparisons and guidance to help patients maintain stable recovery outcomes.
Dr. Wang Liang presented multiple clinical cases, including keloids of different sizes and locations. The pre- and post-operative comparisons showed improvements in the flatness and color of the scars after the grafting. He stressed that this technique is suitable for certain patients and not all scar types, and individualized treatment plans should be developed after a doctor's assessment.
There are various surgical approaches for treating keloids, and in-situ autografting is one of them. Bakaang Medical has been exploring the clinical application of this technique for many years, developing a complete process from surgery to postoperative management and long-term follow-up.
